I visited the Rapid Response Community Service, which is an exciting pilot project in Warrington bringing together various services from the NHS, social care and the Council under one roof to allow them to provide urgent whole-person support to anyone in the community who needs it within a target time of 2 hours.

This helps to reduce the pressures on A&E and the ambulance service, as well as providing support to ensure people ready to be discharged from hospital have the care they need to go home (and to address the issues that brought them in, working with Warrington Disability Partnership and others on adaptations to homes and with repair people to fix things that are acting as a barrier to safe, independent living).

The range of support they can provide is really impressive, and is an excellent model for multidisciplinary and multi-agency support that stops people falling through cracks in the system.
